Set Health provide individually catered Diabetes Education, Management and Care for patients and their support network. Our immensely knowledgeable diabetes educators will analyse all aspects of your living circumstances that have an effect on your health and put a plan in place to put you on the right track for good health.
It’s vital for patients to have rigid support systems, that’s why we also offer training for Families, Carers and Support workers alike.
We provide information on the following topics and more:
- Diabetes education – Type 1 diabetes, Type 2 diabetes, and Gestational diabetes
- Blood glucose meters – provision, education and set up,
- NDSS registration
- Insulin commencement and ongoing management,
- Insulin pump therapy education – pump preparation, upgrades and new pump starts, ongoing management and education,
- Smart meters/app on phone – to assist with managing insulin when counting carbohydrates,
- Continuous Glucose Monitoring – set up, education, interpretation
- Preconception planning for people living with diabetes,
- Gestational diabetes – education and ongoing management,
- As well as ongoing patient management.

- DAFNE (Dose adjustment for normal eating).
NDIS– Set Health help patients with disabilities develop strategies to remove barriers created by their disability. We empower individuals with diabetes, their family and care team to live free from the anxiety which surrounds diabetes management.
The aim is to build the capacity of NDIS participants, and their circle of support, to improve the ability of the person to have their diabetes care attended to. Everyone with diabetes should have the support they need to feel confident in their self-management.
Training, Diabetes Education and Support for Families, Support Workers and Carers.
We formulate a training package specifically suited to the needs of the person(s) with diabetes.
Set Health coordinates with the client, family and any relevant health care professionals, to create a training session that will prepare the staff to be able to care for the person(s) with confidence.
Our sessions are mobile and able to be delivered on-site at the clients home or your place of business. They are 100% hands-on and aim to give participants practical and applicable knowledge. All Equipment that the person(s) currently use will be provided to practice with and contextualised resources are provided.
After the training, all staff will be issued with a 12-month certificate of competency for insurance purposes and PDP points.
One of the leading services we provide is our one-on-one telephone assistance and guidance in the event that you have questions after the training. This is useful if you find yourself not knowing what to do in any situation. You can talk with our trainer anytime!
